Rules for School Volunteers and Chaperones
Updated: January 2023
All volunteers (including chaperones) must obtain clearances and background checks are in accordance with the Pennsylvania Child Protective Services Law. No adult will be allowed to volunteer or chaperone at FACTS without these clearances on file.
Use links below to obtain the 3 required clearances
- Pennsylvania State Police Records Request
- Child Abuse Clearance (on-line) or Paper Version of Child Abuse Clearance
- Federal Criminal History Record (FBI fingerprints) or a signed affidavit (affirm in writing) (if you have lived in PA for more than 10 years).

Once you obtain the certifications, please bring the originals to Teacher Patty in the main office who will make a copy and return your originals. Please be assured that all information will be kept confidential. We greatly appreciate your support in helping to provide special experiences for our students.
